Getting started with Flourish is simple. Before your first visit, we'll ask you to complete three short forms. We promise to keep the paperwork as quick and easy as possible—each one helps ensure you're informed, prepared, and ready to get the most out of your care. 

1. Informed Consent.  Know what to expect from pelvic physical therapy and give us your consent to begin care. 

2. Financial Agreement. No surprises. Review our payment policies and other financial details before we get started. 

3. Intake Form. Tell us a little about yourself, your concerns, and your goals. The more we know before we meet, the more we can focus on you during your first visit.
